Wise Family Funeral & Cremation Services

(Effective July 1, 2008)

The goods and services shown below are those we provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic service. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reasons in writing on the statement we provide describing the funeral goods and services you selected. The list does not include prices for certain items that you may ask us to buy for you such as cemetery fees, flowers and newspaper notices. The prices for those items will be shown on your bill or statement describing the funeral goods and services you selected.

BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF

The basic service charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices and coordinating of service plans with parties involved in the final disposition of the deceased. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.

EMBALMING

$595.00 *

Except in special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as funeral with viewing. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for embalming, such as direct cremation or immediate burial.

OTHER PREPARATION OF THE DECEASED

This consists of restorative art cosmetics, dressing of the deceased and placing the deceased in the casket.

FORWARDING DECEASED TO ANOTHER FUNERAL HOME

This charge includes transfer of remains to funeral home, basic services of staff, necessary authorizations, embalming and local transportation, shipping container included. (excluding shipping charges.) (Use of funeral home for visitation is available at additional expense.)

RECEIVING DECEASED FROM ANOTHER FUNERAL HOME

This charge includes basic services of staff and local transportation. (Use of funeral home for visitation or service is available at additional expense.

IMMEDIATE BURIALS

$2195.00

This charge includes removal, care of deceased, preparation and filing of all necessary notices, authorizations and consents and transportation to cemetery. Price does not include casket, vault, embalming or cemetery expense.

DIRECT CREMATION

$2135.00

Simple direct cremation without any service or body present for reviewal. Service includes removal from place of death within a 30 mile radius, preparation and filing of all the necessary notices, authorizations and consent and container for cremation. Alternative container provided.

RENTAL CASKET

$945.00

The casket used for the funeral service may have been used in a previous funeral service. If the casket has been used in a previous service, the interior lining has been replaced.

Certified Copies of the Death Certificate: $13.00 for the first copy and $6.00 for each additional copy

Obituary: A credit card is needed for all out of town papers.

KDWA Radio: $10.00

Flowers: List of flowers and prices available at the funeral home or you may work with a florist of your choice.

Cemetery: We will assist the family with burial arrangements, however, the cemetery may have charges

Cemetery Markers and Memorials: Ask staff for information and pricing.

Motorcycle escort: $225.00

Hair stylist: $60.00

Bagpiper: $175.00 - $225.00

Memorial Videos: The first copy is a gift from Wise Family Funeral Home. Additional copies are $25.00 per copy or 10 or more copies are $20.00. each.
